Team Activation simplified

There was a bug in the edit of a player's date of birth (DOB), and the GLSL did not use this information for any purpose anyway. To eliminate the bug, and to be in accordance with PIPEDA, collection of a player's DOB by the GLSL has been discontinued.

Similarly, a player's OSA# is useful to the GLSL in the Discipline process. However, for the small number of Discipline Hearings the GLSL performs each year we have decided to collect this information at the time of performing Discipline, rather than have every coach enter the OSA# for every player.

Note that after the season starts the GLSL does send team player rosters to each of the club, for comparison against the registration systems. Any coaches using unregistered players for any game will be submitted for discipline by the OSA (using an ineligible player is a 3-12 month coaching suspension).
